- I understand that the facilitator, Aida Bryant / Karuna Flow, provides wellness and energy-healing services, including distance Reiki, sound healing, mixed modalities and personal alignment coaching, and these services are not medical treatment, not therapy, not diagnosis, and not a substitute for mental health or medical care.

- I acknowledge that I have been informed that in the State of Nevada, a person providing wellness services who is not licensed, certified or registered as a provider of health care must disclose that fact. 

- I confirm I have been told that the provider is not licensed as a medical doctor, psychologist, or other licensed health-care professional.

- I understand I am responsible for consulting my primary licensed health-care provider before doing anything that may affect my health.

- I understand that the nature of the session is energetic/spiritual/wellness-oriented, and that results are not guaranteed and will vary from person to person.

- I understand that I may experience emotional, mental or physical releases or shifts during or after a session; I agree to care for myself post-session (rest, hydrate, reflect) and seek appropriate professional care if needed.

- I agree to provide accurate information, keep my appointment, and complete any intake or feedback forms requested by Karuna Flow.

- I understand and agree to the Cancellation & Rescheduling Policy (see policy section on booking page) and that failure to abide may forfeit future bookings or require full payment of the session fee.

- I consent to the use of online video, phone or distance healing as applicable. I understand that by participating I accept the limitations and risks of remote sessions (such as internet connectivity, privacy in my space, etc.).

- I grant permission to Aida Bryant / Karuna Flow to send session reminders and (if applicable) optional follow-up check-ins via email.

- I understand that my personal data will be handled according to the Privacy Policy (available on the website) and will not be shared without my explicit consent, except as required by law.

- I understand that by clicking “I Agree” (or signing electronically) I affirm I am at least 18 years old and have read, understood, and consent to this form.
